On biopsy we appreciate a usual interstitial pneumonia pattern with patchy fibrosis accompanied by honeycomb change, consistent with a diagnosis of UIP. Discordant radiologic and histopathologic findings reinforce the need for a multidisciplinary discussion among the pulmonologist, radiologist, and pathologist to arrive at a consensus diagnosis.
