As far as connective tissue disease, does this patient have rheumatoid arthritis? She has a negative rheumatoid factor and, if she has a negative aCCP, it would seem highly unlikely. So given the age, the gender, and the comorbidities coupled with the physical exam and CT findings, we would move down a path of scleroderma.
